:: How does Xenical work?
When you eat more fat or calories than your body needs, it is stored as fat by your body, resulting in weight gain.
Xenical does not enter the blood stream. Instead, it works in your digestive system to block about one-third of the fat in the food you eat from being digested and absorbed into your body.
Enzymes in your digestive system, called lipases, help digest (or break down) fat so your body can absorb it. By attaching itself to these lipases, Xenical blocks them from breaking down the fat in your meal thereby reducing the amount of fat that is available to enter your system. The undigested fat is then eliminated in your bowel movements.
In other words, you eat more calories than are absorbed by your body. As you maintain this lower calorie diet, you allow your body to use your stored body fat, and you lose weight.

There is clear scientific evidence that if you are overweight, losing as little as 5% of your body weight can result in improved health.
In clinical studies over a 52-week period, people treated with Xenical and a reduced-calorie diet, with no more than 30% of calories from fat lost an average of twice as much weight as those who used the diet alone (approximately 13.4 lbs versus 5.8 lbs). Almost 70% of the patients lost 13.4 pounds after three months and 19 pounds after one year.
Weight loss with Xenical and a healthy diet also improves several weight-related health risks. Weight loss with Xenical can:
- Decrease total cholesterol and LDLs (low-density lipoprotein) also known as bad   cholesterol.
- Improve the ratio of bad to good cholesterol (LDL/HDL cholesterol).
- Reduce blood pressure.
- Reduce levels of insulin (hormone that allows glucose or sugar to enter the body's   cells), high levels of which are associated with coronary heart disease.

Xenical (Orlistat) has been used to treat obesity since 1999. Its safety profile has been maintained by real-world experience as well as clinical trials.
The effects of Xenical on weight-loss and weight-loss maintenance, as well as the effects of weight loss with Xenical on other weight-related health problems (such as high cholesterol levels, type 2 diabetes and high blood pressure), have been evaluated in seven 1-to-2-year, multicenter, double-blind, placebo-controlled studies which included more than 4100 patients.

:: How soon after starting Xenical should I expect to notice       weight loss?
In most cases, you should begin losing weight within 2 weeks of starting Xenical, and may continue to lose weight for 6 to 12 months.
After 6 to 12 months, your weight-loss will probably start to level off, even if you’re still taking Xenical. This perfectly normal event called plateauing does not mean that Xenical is not working. It means that your new leaner body now needs fewer calories to maintain itself. :: Why have I stopped losing weight after following the Xenical plan?
This is known as plateauing or leveling-off, and it's perfectly normal. When this happens, you need to re-adjust your calorie restriction and/or your activity level.

A plateauing or leveling-off of your weight-loss means your new leaner body needs fewer calories to maintain itself than when you started the Xenical program.
For example, if your starting weight had a 2500 calorie per day requirement to maintain itself, and you reduced your calorie intake by 250 calories per day to lose about half a pound per week, after 6 months of eating 2250 calories per day (2500 minus 250), you should weigh about 12 pounds less and your body now would need approximately 2400 calories per day to maintain itself and 2150 calories per day (2400 minus 250) to lose that half a pound per week. However, you are still on the initial 2250 calories per day plan. It now takes more than twice as long to lose that half a pound, your weight-loss is beginning to plateau. To lose more weight at the same rate you have to increase exercise (to burn calories) and further decrease calorie intake.

You may also experience plateauing or leveling off with your weight-loss if you've been increasing your physical activity level. If you have been building muscle and losing fat, the weight from fat is replaced with weight from muscle. Muscle weighs more than fat, so your body measurements (clothing size) may be decreasing but you don’t see a change on the scale. You may want to maintain a record of your body measurements, like your waist-size so you can properly evaluate your results.

It is likely that you will experience some changes in bowel habits. The side effects generally occur during the first weeks of treatment and may go away during treatment as your body adjusts to Xenical.
For some people however, the side effects may continue for 6 months or longer while on Xenical. These changes may include gas with oily discharge, an increased number of bowel movements, an urgent need to have them, and an inability to control them, particularly after meals containing higher amounts of fat than are recommended.

:: What other precautions should I take when using Xenical?
If you are taking cyclosporine, be aware that Xenical and cyclosporine should not be taken within 2 hours of each other. You may need more-frequent monitoring of cyclosporine levels if you are taking both drugs.
Xenical reduces the absorption of vitamins, therefore, a daily multivitamin is recommended.
Do NOT share your Xenical prescription with others; this can be a very dangerous practice.

Inform your doctor, during either a physical or online consultation, about all medications you are currently taking, prescription and non-prescription, to determine if Xenical can be taken with these medications.
Let you doctor know if you are taking cyclosporine before starting Xenical. Xenical and cyclosporine should not be taken within 2 hours of each other. You may need more-frequent monitoring of cyclosporine levels if you are taking both drugs.
Let you doctor know if you are taking Warfarin before starting Xenical. Xenical does not affect warfarin directly but may decrease the absorption of vitamin K (vitamin involved in coagulation). You may need more-frequent monitoring for changes in coagulation parameters.
Clinical studies have shown that Xenical does NOT interact with the following commonly used medications: digoxin, gyburide, nifedipine, oral contraceptives (birth control pills), phenytoin, and pravastatin.

:: Can I take Xenical and other weight loss drugs at the same time?
Xenical (orlistat) works by blocking the digestion and absorption of fat to help you control your fat intake. Xenical is a non-systemic drug, therefore there are no drug interactions between Xenical (orlistat) and centrally acting weight loss medications that suppress appetite like
Phentermine ,
Adipex ,
Ionamin ,
Bontril ,
Didrex ,
Meridia , or
Tenuate . This would seem to make Xenical a good choice to use with existing medications to increase the weight loss effects.
Unfortunately, studies conducted so far do not indicate that Xenical has an increased effect on weight loss when combined with other drugs. The combinations of Xenical with phentermine and Xenical with Meridia (sibutramine) have both been tested in pilot clinical trials with minimal results. Larger studies need to be conducted on these combinations before any definitive conclusions can be made.
Xenical is not indicated for use with other weight loss medications because the safety and efficacy has not been studied. Taking Xenical with phentermine or Meridia would be considered an off-label use, meaning that it hasn't been approved by the FDA.
However, the FDA has approved both drugs separately, and once the agency has approved a drug, doctors may prescribe it as desired. Often combination therapies are not studied because since each individual drug has already been approved by the FDA and is on the market, the cost of the study does not directly lead to the launching of a new drug. The lack of studies does not imply the combination is unsafe, but it also does not necessarily support its safety either. The safety of the combination is established over time as patients utilize the medications.
Therfore, your physician may determine that you may safely take Xenical with other weight-loss medications for a more efficient weight-loss. Prior to this determination you must inform your healthcare provider of your complete medical history, including any other medicines you may be taking (both prescription and non-prescription).
